New Patients:
- Please arrive at least 20 minutes prior to your scheduled appointment time to allow time to check in. Please be sure to fill out the forms online which were sent to you via email when your appointment was scheduled.
- Be sure to bring photo ID, all applicable insurance cards and referrals if your plan requires one. If you have more than one insurance plan, all cards must be presented at the time of your visit (i.e. primary and secondary plans, medicare and/or medicaid cards).
- Please bring a list of all your current medications and the name and phone number of your primary care physician and pharmacy.
Referrals:
It is the patient's responsibility to know if a referral is required by their plan and to obtain the necessary referral(s) from the primary care physician. If the proper referral is not provided, your insurance plan will not cover services and you will be billed for the visit.

Appointments:
Our office makes every effort to contact you before your appointment to confirm it . Referrals, if required, are the patient's responsibility to obtain in advance of the appointment.
We make every effort to accommodate urgent and emergency appointments as well as our regularly scheduled patients in an efficient and expeditious manner. However, from time to time, there are un-avoidable delays. During these times, we appreciate your patience and will do our best to ensure a pleasant and timely visit. Masks are still required in our office and all healthcare facilities.
Existing patients will be asked to update patient forms annually so that we have current information and signatures on file for insurance requirements. For your convenience, patient forms will be sent via email and/or text message.
Please bring with you to your appointment the following items if applicable: any X-rays, Mri's and diagnostic imaging disks or films and reports, a current copy of your insurance card, photo identification, referral and a method of payment: cash, check or major credit card. ALL COPAYMENTS MUST BE PAID AT THE TIME OF VISIT. NO EXCEPTIONS CAN BE MADE. If a copayment must be billed, a $10 administrative charge will be added to your balance due. If your insurance plan has a deductible that is not yet met, you will be asked to make an upfront payment of $75 towards the amount your insurance company will deem your responsibility. Any overpayments will be refunded or applied to your next visit once all claims have been fully processed. You will be billed for any remaining balance due.

Emergency Appointments
If you need an appointment within 24-48 hours, please call the office at (718) 816-8634. If the office is closed, please leave a message and someone will return your call as soon as possible.

As a courtesy to the Doctor and other patients, we kindly request 24 hour notice for cancellations. If you are more than 15 minutes late for your appointment, you may be asked to reschedule. No-show appointments may incur a fee of $25 in addition to possibly not being given any future appointments. We have great respect for the time of our patients and therefore do not over book the schedule. In turn we ask for the same respect for the Doctor's time. Please call if you cannot make your appointment so we may use that time for another patient.
